To make an appointment, please contact us and a representative will contact you within 1-2 business days to assist you in scheduling your appointment.

• If this matter is of an urgent nature, please contact your primary physician or go to your local emergency room immediately.
  
• The Neuroscience Center at Allegheny General Hospital cannot diagnose or treat by email. Our e-mail policies are explained further in this Disclaimer. You may also wish to review our Privacy and Patient Rights information.
• Aftre scheduling an appointment, please print and complete the patient history form if you haven't already recieved a packet in the mail. There will also be a registration sheet and consent to treatment forms to fill out once you arrive at the office.
